2. Please fill up the GROUP SCORE (Except the Project Head’s and the Instructor)
3. Write the names of your members at the blank area under “Group Members”
4. Evaluate the members according to the five categories: focus,contribution, involvement, presence, attitude. 5 is the highest, 0 the lowest.
5. Don’t give a 5 if that person deserves the 5 score.
6. Sign the Form, Pass this to Maam Tata.
7. Jimmy will get all the forms sa Tuesday
8. Jimmy will pass the forms on Wednesday to Sir Rustom.
